Our TeamCare GA, MD. is looking to provide the best possible services and ensure patients receive the treatment they need, the intake process must be completed.

This process is put into place to determine whether our office can provide comprehensive care.

We ask you to read the steps below before completing our online intake form and be aware that completing the request an appointment form does not guarantee service with our office.

Although there are some cases where we will not be able to meet your needs and will provide you with referrals to other practices.

Please note, that the initial visit is a consultation, and a patient-doctor relationship is not established until both you and your physician decide a treatment relationship is appropriate. If this is an emergency, please call 911.

Intake Process Steps

STEP 1 – BOOK A 15-MIN FREE CONSULTATION

Start by booking a complimentary 15-minute consultation to discuss your needs and how we can help by submitting an appointment form to proceed with your request for a consultation.

STEP 2 – APPROVAL

Within 24-48 hours, you will be notified via email whether your appointment request has been approved or denied. Approved requests will include Patient Portal login details.

STEP 3 – SCHEDULE AN APPOINTMENT

Log in to the Patient Portal, verify your information, and schedule your appointment. We’ll review any previous records as necessary.

STEP 4 – CHECK-IN

Complete all required forms, including registration, privacy policies, clinical forms, and histories. Payment for any applicable fees or balances will be collected at this time. For telehealth appointments, our team will assist with setup and form completion.

STEP 5 – CONSULTATION

New patient evaluations are scheduled for 90 minutes, including a comprehensive assessment, clinical interview, diagnostic clarification, and treatment planning.

STEP 6 – CHECK OUT

Our staff will assist with medication delivery arrangements and schedule follow-up appointments.

By following these steps, we ensure a thorough and efficient intake process, paving the way for a successful and supportive patient-doctor relationship.
